Most uses and disclosures of psychotherapy notes (if applicable), marketing communications, and sale of PHI require your written authorization. You may revoke an authorization in writing, except to the extent we have already acted on it.

• Request confidential communications
• Inspect and copy your PHI (with limited exceptions)
• Request an amendment to your PHI
• Receive an accounting of certain disclosures
• Receive a paper copy of this notice
• File a complaint if you believe your privacy rights were violated

5. Complaints

If you believe your privacy rights have been violated, you may contact us or the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

7. Changes to this notice

We reserve the right to change this notice and make the revised notice effective for PHI we already have as well as any PHI we receive in the future. The current notice will be posted at the clinic and on our website.
